Progressive Rehabilitation: Providing Sub-Acute & Inpatient Care

Our 44-bed sub-acute progressive rehabilitation unit which offers well-appointed rooms with internet access, TV’s, and private bathrooms. Private rooms are also available for patients upon request. A 3,000 square foot, state-of-the-art therapy gym furnished with various equipment used to increase function and independence including a treadmill, Nu-Step machine, standing table, parallel bars, and a recumbent bike. Therapists also use specialized equipment like the LiteGait system, which assists patients who must maintain a limited weight-bearing status and maintains proper posture during sub-acute rehabilitation. A full kitchen, full bathroom, and laundry facilities are also installed in the gym to help mimic realistic activities, further ensuring safety upon returning home.

Our home caregivers come from a diverse background of providing acute, sub-acute, inpatient, and outpatient care. Some have even worked at top research rehabilitation centers. The expertise of our staff is complemented by years of experience, with each of our home caregivers holding anywhere from 10 to 20 years of practice and specialization.

Rated “excellent” in patient satisfaction surveys, our progressive rehabilitation department is committed to assisting patients in reaching their goals. Therapists achieve excellent outcomes by providing personal attention to each patient, addressing functional transfers and mobility issues, practicing “everyday” activities, and providing patient/caregiver education to ensure success upon discharge. “Family meetings” are arranged with our therapists along with our social services department to discuss patient’s progress and answer any questions or concerns families may have regarding the patient’s rehabilitative goals. The team of sub-acute rehabilitation home caregivers includes a physiatrist who visit weekly to medically oversee the rehab program and ensure patient recovery. Programs run 7 days a week and include occupational, physical and speech therapy.

Hospice Care Services

In collaboration with several hospice providers, the Inglemoor facility provides inpatient hospice care services.Inglemoor’s focus has always been to maintain comfort and dignity for all residents at all times, and this particular stage in life is no exception. Coordinating with hospice has given the facility the tools necessary to hold training in-services for the staff, to assess the medical and emotional needs of residents, and to provide a strong support system for their families. The interdisciplinary team works with hospice staff and families to create a care plan unique for each resident.

Inglemoor is a 130-bed skilled nursing facility that provides both sub-acute rehabilitation and long-term care. Inglemoor maintains its mission, “Excellence in Service,” which drives the staff to strive for high-quality outcomes and innovative health care initiatives. Inglemoor houses two separate units for sub-acute and long-term care, enabling staff members to provide consistent assignments and meet the specialized needs of the facility’s different populations.

Every patient is treated as an individual and given a specific care plan based on his/her particular medical and rehabilitation needs.
